I'm pretty sure it doesn't come with the slip any more.

And yes, it's region A locked unfortunately.

It'd be very hard to find this slipcover still, and if you do, it'd most likely be in bad condition. Back when it came out, it was hard to find a good-looking one. Due to the design of it, the ones in stores were often ripped.

Yeah, they definitely came with the slipcover, but they were very scarce. I rarely saw them in-store and when I did, they were often damaged. Most stores only sold the complete box set:
