I'm going to get the lenticular because I plan on opening mine, but if you want a sealed copy, I would personally go with the slip. Still, it has got to be up to you!

That is a great question. It does not have a field for state. No where in my address does it indicate CA. Of course, my 5 digit zip code is linked to CA, but still, it is odd.

I just put a comma next to city, then "CA".
