Merchandise $20. Declare invoice as same value and choose k packet.

You keep it under the customs value threshold. Marking it as a gift will not work, as the outside packaging confirms you purchased from a retailer

Since you're in the US, you won't be hit by customs for such a low amount (the full steelbook's value). Those posts you mention were mainly for/by members in the UK where they have different customs regulations.

If you're in the US, you will be fine. If in the EU, then a customs friendly value is recommended

Yes, that is what it appears. Limit 2 one-clicks per order. No limit on the individual editions.
