Theres a Blu-ray and a DVD version. I believe the Blu-ray version is very hard to find though they are both OOP now.

It came attached to the back with a red cardboard like header advertising it.

I have the graphic novel comic, I can take pics later. Xage has it brand new sealed still so it appears.

It's not from the US, it's from Austria. The movie was released by 84' Entertaiment, the same label that also released the Hellraiser 4 mediabook.

This Mediabook is limited to 500 pieces and since a few months out of print.
